
Second Installment Voucher
Dear Student,
When making the payments please follow the following guidelines.
- Indicate the branch name of the bank very clearly on the space provided in the voucher.
- Make sure that the “Voucher Reference Number” stated in your payment voucher has been entered in bank payment documents of the Bank when you make the payment. This reference number is very essential to credit the amount into your personal student’s account maintained by the University.
- Indicate the date of the payment and place your signature.
- Make sure that the signature of the Cashier and the rubber stamp of the Bank are clearly placed in the given space before you leave the Bank.
After paying the second installment, please upload the copy of the paid deposit slip together with the University copy of the voucher which authorized by the bank.
